The People Who Feel Every Touch They See

In 2005, a woman sat down in a laboratory at University College London and watched video clips of people being touched. A hand was stroked. A cheek was brushed. Nothing happened to her at all, and yet she felt every touch on her own body, in the corresponding place, at the exact moment she saw it.

The case was written up by Sarah-Jayne Blakemore and her colleagues in the journal Brain, under the dry clinical label "vision-touch synaesthesia." We know it now as mirror-touch synaesthesia. In the two decades since that paper, it has become one of the most quietly useful conditions in neuroscience — not because it is an illness, but because it is a window.

The phenomenon comes in two forms. In the more common one, the sensation is specular, meaning it behaves like a mirror: if you watch someone being tapped on their left shoulder, you feel it on your right. In the other, it is anatomical, landing on the same side of your body as theirs. Either way the sensation is not imagined or remembered. It is felt on the skin, with a location and a quality, the way a real touch is felt.

For most of us, seeing is not a passive act. When you watch another person move, your own motor cortex quietly rehearses the movement. When you see a hand close around a cup, the muscles in your own hand receive a whisper of a command that never quite reaches them. This is the mirror system, and it is not a rare gift. It is standard equipment. What makes a mirror-touch synaesthete unusual is not that the simulation happens but that it is not properly fenced off.

The leading explanation runs like this. In a typical brain, the sensory simulation triggered by watching someone else is tagged as belonging to somebody else. The tag is what stops you from flinching every time a stranger stubs a toe. In mirror-touch synaesthesia, that tagging is weaker, and the simulated signal is allowed to spill over into the primary somatosensory cortex — the strip of brain that maps touch across the surface of your body — where it is processed as though it had come up the nerves from your own skin.

The brain imaging supports this. In the original case, the woman's scans showed overactivity across the tactile mirror system, including the somatosensory cortex, the premotor cortex, and the insula — the same circuitry that in everyone else is kept below the threshold of conscious sensation. The part of the brain that maps touch across your own body appears to be doing the work of mapping other people's. When Ward and Banissy tested a group of mirror-touch synaesthetes in the laboratory, they were measurably better than controls at detecting a touch delivered to someone else's face, and they made characteristic errors, reporting a touch on both cheeks when only one had been touched.

Michael Banissy and Jamie Ward, both then at UCL, went on to ask a harder question: if you literally feel other people's sensations, are you better at understanding them? In a 2007 paper in Nature Neuroscience, they reported that mirror-touch synaesthetes scored significantly higher than controls on standard measures of empathy, particularly on emotional reactivity — the tendency to be moved by what happens to other people. Later work estimated that about one and a half percent of people have the trait, which means that in a lecture hall of two hundred students, three of them are feeling the door slam on somebody else's fingers.

There is a cost, and the people who have it tend to describe the cost before they describe the gift. Crowded trains are difficult. Horror films are intolerable. Watching a nurse take blood, or a friend receive an injection, produces a genuine physical sensation that cannot be switched off by reminding yourself that it is not happening to you. Some describe a blurring of the boundary between their own emotional state and the mood of whoever is in the room, a kind of porousness that is exhausting by the end of a long day.

There is a relative condition, mirror-pain synaesthesia, in which the sensation is not touch but actual pain — a stab or a burn that arrives when you watch someone else injured. Estimates of how common it is vary enormously, and many people who have it have never mentioned it to anyone, on the reasonable assumption that everyone experiences the same thing and simply declines to discuss it.

It is worth saying that none of this makes mirror-touch synaesthetes saints or mind-readers. They are not more moral than anyone else. What they have is a dial turned further up on something everyone possesses. If you have ever yawned because someone else yawned, or felt your own leg twinge when a footballer's knee buckled, you have the same machinery at a lower setting. The question the research leaves open is why the setting varies at all — and whether the world is better served by a nervous system that holds other people at a distance, or one that insists, in the most literal way available, that the boundary between you and me is thinner than we like to think.

Feeling what someone else feels is not a metaphor for some people. It is a report from the skin.
